Your initial visit is an opportunity to meet the physician, review your medical history, and determine whether an ongoing physician–patient relationship is the right fit for your care.
Please bring one main concern or problem to your regular appointment. If you have multiple issues, additional appointments may be required to address them.
Your annual physical is designed to keep you healthy and help with early disease detection. If new concerns come up, they may need their own appointment—we recommend booking as soon as you notice an issue instead of waiting until your yearly exam. Please give us at least 24 hours’ notice if you need to cancel. Missed appointments without notice may result in a no-show fee.

All phone calls are managed by our clinic staff. If you would like to speak directly with a physician, please book an appointment.
Patients will be contacted if follow-up is required based on test results. Please note that lab and other test results are not provided over the phone. If you have ongoing concerns about your testing, we encourage you to book a follow-up appointment with your physician. This appointment should be scheduled at least one week after testing, to allow time for results to be received and reviewed.
Your physician will consider accepting and renewing a prescription sent to our office by your pharmacy if you have regularly attended your office visits regarding your healthcare concerns.
