____ New Patients ____
Your first appointment is primarily to meet the physician, review your medical history and decide whether you should have an ongoing physician and patient relationship.
____ Appointments ____
You should generally bring a single problem for your physician to address during a regular appointment. Multiple problems will require additional appointments.
__ Annual Physicals __
The purpose of annual physicals is health promotion and disease screening. New problems may require additional appointments. We encourage you to book an appointment when the problem arises, rather than saving them up for the annual exam.24 hours notice is required for appointment
cancellations. There will be a no-show fee charged
for missed appointments.

__ Phone Calls __
All phone calls are handled by clinic staff. To speak to a physician an appointment must be made.
__ Lab Results __
Patients will be notified in the event follow up is required as a consequence of testing. Lab or other test results are not available over the phone. If you have on-going concerns in regard to the testing that was ordered we encourage you to book a follow-up appointment with your physician to review your results. This appointment should be scheduled no earlier than one week post testing to ensure that your results have been received and reviewed.
___ Prescription Refills ___
Your physician will consider accepting and renewing a prescription sent to our office by your pharmacy if you have regularly attended your office visits regarding your healthcare concerns.
___ Same Day Clinic & Walk-In Clinic ___
We provide a same day clinic as well as a walk-in clinic to patients of our clinic as well as to the general public. Please call our office or attend to reception to arrange an appointment. Please restrict yourself to one problem during these appointments.
___Additional Services Offered ___
In addition to Primary Care Healthcare services, our center offers onsite Pharmacy, Workplace Related Injury services, Wart Treatment, Skin Tag removal, Surgical Skin Lesion Removal, etc. If the service that you are looking for is not listed above, please speak with your physician about the many other services that are offered by our center.
___ Un-insured Services ___
Please note that some services* provided by our clinic are not covered by your provincial health insurance plan. Please consult our list of services and the charges applicable to each. These costs are your responsibility. (*for example: sick notes, insurance forms, travel vaccinations, driver’s physicals, transfer of medical records, etc.)
